AG Bewegungsstörungen und Tiefe Hirnstimulation
Die Arbeitsgruppe für Bewegungsstörungen und Tiefe Hirnstimulation beschäftigt sich hauptsächlich mit der Frage, wie die Effekte der Tiefen Hirnstimulation (THS) bei Patientinnen und Patienten mit Bewegungsstörungen, vor allem bei Morbus Parkinson, bei Essentiellem Tremor und bei Dystonie, optimiert werden können. Hierzu führen wir sowohl monozentrische als auch multizentrische Studien durch. Die Beurteilung der Therapieeffekte erfolgt anhand von Querschnitt- und Längsschnittstudien. Ein weiterer Schwerpunkt liegt darauf, die Effekte der Tiefen Hirnstimulation für jede Patientin/ jeden Patienten individuell vorhersagen zu können.
Einen besonderen Fokus legen wir auf die Analyse von Bildgebungsdaten, also MRT und CT-Bildern. Moderne bildgebende Verfahren ermöglichen es uns die genaue Lage der Elektroden der THS im Gehirn zu bestimmen und zu erkennen, welche neuroanatomischen Regionen und Netzwerke bei individuellen Patientinnen und Patienten stimuliert werden. So können wir herausfinden, wie die Wirkungen, aber auch mögliche Nebenwirkungen der THS entstehen. Mit diesem Wissen versuchen wir langfristig die individuelle Stimulationseinstellung jeder Patientin und jedes Patienten zu optimieren.
Häufig treten stimulationsinduzierte Sprechstörungen als Nebenwirkung der THS auf, welche für uns von besonderem Interesse sind. Bei Patientinnen und Patienten mit Bewegungsstörungen untersuchen wir inwiefern sich Sprechparameter (Stimme, Lautstärke, Artikulation, Prosodie) durch die Hirnstimulation verändern. Dafür führen wir in Kooperation mit Prof. Dr. Doris Mücke der Universität zu Köln (Phonetik) sowohl akustische als auch artikulatorische Sprechmessungen durch. Seit 2019 ist Prof. Michael Barbe Mitglied der MDS Speech and Movement Disorders Study Group und hat dadurch die Möglichkeit, in einem großen internationalen Rahmen aktuelle Forschungsprojekte voranzutreiben.
Die AG Bewegungsstörungen und Tiefe Hirnstimulation beteiligt sich an der Spezialambulanz Bewegungsstörungen zur diagnostischen und therapeutischen Mitbeurteilung von ambulanten Patientinnen und Patienten mit Bewegungsstörungen (v. a. Parkinson- und Tremorsyndrome, Dystonie, choreatiforme Störungen). Schwerpunkte sind hierbei eine „zweite Meinung“, Medikamentenoptimierung, ausführliche Aufklärung über Therapieoptionen sowie die Beratung im fortgeschrittenen Stadium auch hinsichtlich invasiver Therapieverfahren, wie z.B. die Tiefe Hirnstimulation und Medikamentenpumpen. Zudem erfolgt die ambulante Nachsorge bei bereits bestehender Tiefer Hirnstimulation in unserer Spezialambulanz.
Über das Kölner Parkinson Netzwerk (KPN) wollen wir im Dialog mit niedergelassenen Neurologinnen und Neurologen eine optimale Versorgung unserer Patientinnen und Patienten gewährleisten und aktuelle Erkenntnisse unserer Forschung in die Patientenversorgung einfließen lassen. Unser Netzwerk besteht aus ca. 30 niedergelassenen Neurologinnen und Neurologen, mit denen wir in regelmäßigem Austausch stehen und gemeinsam Fortbildungen durchführen. Die Patientinnen und Patienten haben die Möglichkeit, sich in unserer Beratungsstelle bei unserer Parkinson Nurse Susanne Hoffmann über das KPN zu informieren. Zudem organisieren wir jährlich das Seminar für Patientinnen und Patienten mit Morbus Parkinson, um eine Informationsmöglichkeit über aktuelle Forschung sowie neue und bewährte Therapien zu bieten. Die lokalen Kooperationspartner unserer Arbeitsgruppe sind die Klinik für Stereotaxie und Funktionelle Neurochirurgie (Univ.-Prof. Visser-Vandewalle), das Institut für Phonetik der Universität zu Köln (Prof. Dr. Mücke) sowie das Forschungszentrum Jülich (Prof. Dr. Tamgüney). Wir arbeiten zudem eng mit anderen Arbeitsgruppen aus der Neurologie, Psychiatrie und Nuklearmedizin zusammen sowie mit dem Studienbüro der Neurologie.
In einem aktuellen Projekt möchten wir in Kooperation mit dem Forschungszentrum Jülich eine Biodatenbank für Patientinnen und Patienten mit Parkinsonsyndromen und anderen Bewegungsstörungen aufbauen. Hierbei sammeln wir verschiedene Körperflüssigkeiten (Blut, Urin, Speichel etc.) und untersuchen diese nach möglichen Markern, die mit einer Parkinsonerkrankung assoziiert sein könnten.
